Catalyst Therapy has a responsibility to protect the privacy of your personal information.   We will only collect information that is relevant to the services we provide.  We will try to keep information you provide safe. 
You have the right to tell us who we can collect information from and how your information is used.
In handling your personal information, we will comply with the Queensland Information Privacy act (2009).
We have policies and procedures in place to ensure that:
  • we tell you how we manage personal information.
  • your personal information and the personal information of staff are protected.
  • we collect and handle personal information fairly
  • we only collect information we need.
  • we only share information with people who need to know it.
  • we keep your information stored safely

The kinds of information we collect

  • name, address, telephone number and email address
  • date of birth
  • gender
  • advocate or emergency contact’s telephone number and email
  • disability or health information

How we collect personal information

  • your family or support network
  • other therapists or providers
You have the right to let us know who you are happy for us to talk to.
You can withdraw your consent at any time by contacting us.

Disclosure of personal information

​If you or your decision maker consent, we may disclose personal information to other people who support you. 
You have the right to identify who we can provide information to.

Security of personal information
​

​We will take reasonable steps to ensure that personal information is safe and secure.  We hold hard copy and electronic communication.  All electronic communication is stored securely with servers based in Australia. 

Privacy complaints
​

If you have a complaint about privacy we will treat it seriously.  We will deal with it quickly.  If you have a complaint, let us know first.  You may raise complaints with the NDIS Quality and Safeguards commission.
